Payment is required to be credentialed by Memorial Hermann. Fees are payable using a Visa, MasterCard, Discover, American Express or electronic check. The payment process includes your billing information, email, credit card information or bank routing and account numbers for ACH/eCheck (Automated Clearing House/electronic check).

Memorial Hermann surgery and endoscopy centers require a paper check payment to be mailed directly to the facility. To locate a surgery and endoscopy center facility’s address, visit Memorial Hermann's website.
